Request Appointment
To schedule an appointment over the phone, please call (907) 343-4799. If you use the form below to request an appointment, someone at our office will be in touch as soon as possible to finalize an appointment time that works for you. Clinical services are not free, but we offer payment plans, sliding fee scale, and credit/debit payments.
Appointments are strongly encouraged. Please arrive on time. As a courtesy to our other clients, we will have to reschedule you if you arrive 10 minutes past your scheduled appointment time. Last appointment at 4:30pm.
Types of services and when you can schedule an appointment:
- Schedule up to 1 week in advance:
- Initial Exams and Annual Exams
- Schedule up to 24 hours in advance:
- Sexually Transmitted Infection Evaluation and Treatment
- Birth Control Refills
- Pregnancy Tests
- Blood Tests for HIV, Hepatitis and Syphilis
Walk-in clients are offered the next available appointment.
