Please leave a message on our Test Result Message Line at 907-343-4211 and a nurse will get back to you within 24 hours.
Medical Records or Request of Release of Records
Please call 907-343-4792 or use our Authorization Form if you would like your medical records released to you or to another medical facility. Please complete and bring the form to the Anchorage Health Department medical records office.
